By completing this form, you are starting the enrollment process by entering your student into the AIM (Achieving Independence & Maturity) lottery, but your student will not lose their place at their current school. Please make sure to check the accuracy of your answers before submitting. Only one application per student will be accepted.  

Your student must not have taken their diploma their senior year of high school in order to attend the transition program. Your student's current IEP team must have determined eligibility for a transition program.
