Please note that this form WILL NOT evaluate you, but gives us a better idea of your tennis background. We typically host evaluation clinics once a month on Sundays. Filling out this form will not guarantee you a spot in this month's evaluation clinics as limited and filled on a first come-first served basis. Evaluations will only be offered to members.
