City of Edmonton Youth Council Mental Health Survey
One in three Canadians experiences mental health issues in their lives, whether it be directly or indirectly through family, school environment, or friends. Mental health issues often manifest in adolescence and young adulthood but can be feared or misunderstood and left untreated.

We know that we aren't the experts on mental health. We need your help.

The City of Edmonton Youth Council Health and Wellness Committee brings to you the mental health survey. The purpose of this survey is to identify gaps in youth mental health and relay this information to the city council. By presenting this information to the city council we hope to provide more accurate information about youth mental health in our community

Please take a few minutes to complete this survey. All your responses will be used for the purpose of the information report only and are confidential. Your opinion as Edmontonian youth is incredibly valuable and our aim is to ultimately act on the gathered information to implement positive change that directly affects those who need it.

At the end of the survey if you want to enter to win a prize, you can compete a separate confidential form.

The personal information collected through this survey will be used to collect information on mental health, and to provide you with a copy of the report of the data. The collection of this information is authorized under Section 33(c) of the Freedom of Information and Protection of Privacy Act and the privacy protection provisions of that Act will protect it.
