Ivy Prep Academy Bus Transportation Application - 2024-2025 School Year
The 24-25 school year is quickly approaching. Thank you to everyone who participated in our Bus Transportation Survey. The responses were used to plan the bus locations for the school year. Our central location transportation service will consist of pickup/drop-off locations in three areas. 
Please read thoroughly:
  • Families will be responsible for transportation to and from the central locations at the noted scheduled time
  • Please note, due to limited bus space, scholar selection will be on a first come, first served basis. 
  • Completing the application does not guarantee seating for your scholar(s).
  • All applicants will be notified on Tuesday, July 16th.The notification will indicate one of the following
                     - Your scholar has been selected to ride the bus (detailed bus information will be included)
                     - Your scholar has been placed on the waitlist

Bus service will begin on the first day of school, August 1, 2024. 

In order to have your scholar(s) utilize our bus transportation service, YOU MUST COMPLETE THIS FORM, READ AND AGREE TO THE BUS TRANSPORTATION SELECTION PROCESS AND CODE OF CONDUCT. The document is included in this form and included as an attachment. If you have more than one scholar, please complete one form for each scholar. Your application will not be accepted if multiple scholars are included on one form. 

Once again, spaces are limited at each location. If your scholar is not offered a seat initially, she will be placed on a waitlist for your specified location. All siblings will remain together.

Liability Waiver: Please read this section carefully and be aware of its contents to be considered for the Ivy Preparatory Academy Transportation Services.

I will be expressly assuming the risk and legal liability and waiving and releasing all claims for injuries, damages or loss which may be sustained as a result of said services, including but not limited to, vehicle operations and boarding and exiting the vehicle. 

I recognize and acknowledge that there are certain risks of physical injury to vehicle passengers, and I voluntarily agree to assume the full risk of any injuries, damages or loss, regardless of severity, that may be sustained as a result of participating in any and all activities connected with or associated with receiving transportation services, including, but not limited to, injuries, damages and loss arising out of negligent operation or supervision of the vehicle. 

I further agree to waive and relinquish all claims I may have (or accrue to me) against Ivy Preparatory Academy, including its respective faculty & staff (hereinafter collectively referred to as "Party"). 

I do hereby fully release and forever discharge the Party from any and all claims for injuries, damages or loss that may may accrue to my scholar and arising out of, connected with, or in any way associated with said transportation services. 

I further agree that this agreement shall be governed by the laws of the State of Georgia. 

I have read and fully understand the above waiver and release of all claims.
