25-26 Dance Registration Form

Register here for dance classes at MT Ed, open to ages 2 through adult. Please fill out a separate form for each dancer.

Monthly tuition invoices will be sent via QuickBooks to the email address provided. Be sure to enter a valid and frequently checked email to receive all class updates and billing information. (This email cannot be a public school email.)

This form collects basic contact, emergency, and class selection information. If you have any special notes or considerations, there is a space provided at the end.
