If you are a Graduate Assistant, Adjunct Faculty member, Full-Time Faculty member, Professional Staff member, or Student Employee, please let us know your English Department access needs by selecting all the FMH spaces that apply. If you are a returning employee that has not worked at PSU for longer than a year, please make sure to resubmit an access request to the department using this form.
Please note that the Campus Public Safety Office (CPSO) cannot activate your keycard without you first acquiring a physical ID Card at the ID Services window (located on the first floor of FMH).
Once you have acquired your physical ID card, you will be able to complete this form. Feel free to email Department Operations with any follow-up access issues or questions (eng@pdx.edu).
