Participation in Camp is voluntary and is not required, or part of the school. You are in the best position to decide whether or not your child should participate. By signing this form, you agree that your child may participate in camp and all activities taking place during the event. By signing this form, you also agree, as more fully set forth below, that the Church and the School shall not be liable for the negligence of others and that you will hold harmless and indemnify the Church and the School for damages arising to your child during event.

I acknowledge and agree that the Glyndon United Methodist Church, the Glyndon Christian School, and their respective officers, directors, trustees, agents, employees and assigns shall not be liable for any injury to my child resulting from the negligence, errors and/or omissions of others. I further grant my permission for my child to receive emergency medical treatment. I do hereby agree to hold harmless, defend and indemnify the Glyndon United Methodist Church, the Glyndon Christian School, and their respective officers, directors, trustees, agents, employees and assigns from any and all claims, demands, damages, actions, causes of action or suits of any kind or nature whatsoever arising from the actions of my child or my actions in the course of or related to camp.
