By signing up, attending and/or participating in dance classes, dance party, or any event or program, current or future, organized by Jazz Attack / the Philadelphia Swing and Jazz Dance Association (JA/PSAJDA) at any location or online, you hereby agree to the following:

1. You recognize that dancing requires physical exertion that may cause physical strain or injury, and you are fully aware of the risks and hazards involved. You have explained these risks to any minors you accompany.

2. You represent and warrant that you, and any minors you sign up and/or accompany, are physically fit and have no medical condition that would prevent your full participation in the dance classes, dance party, or any event or program.

3. In consideration of being permitted to participate in the dance classes, dance party, or any event or program, you agree to assume full responsibility for any risks, injuries or damages, known or unknown, which you, or the minors you sign up and/or accompany, might incur as a result of attending and/or participating.

4. You authorize JA/PSAJDA to use any photo or video in which you or any minors you sign up and/or accompany may appear for promotional purposes, and waive any rights of compensation or ownership.

5. In consideration of being permitted to attend and/or participate in the dance classes, dance party, or any event, you knowingly, voluntarily and expressly waive any claims you may have against JA/PSAJDA their staff, officers, employees, volunteers, independent contractors, for injury or damages that you or any minors you sign up and/or accompany may sustain, or anything else, as a result of attending and/or participating.

6. You acknowledge that JA/PSAJDA, their staff, officers, employees, volunteers, independent contractors, cannot be held responsible for any loss or theft of your belongings and/or property, at all locations, and that you are solely responsible for them.

7. You acknowledge the contagious nature of COVID-19 and other viruses and voluntarily assume all risk that you, or any minors you sign up and/or accompany, may be exposed to or infected by COVID-19 or other viruses by participating in activities at JA/PSAJDA, and that such exposure or infection could result in personal injury, illness, permanent disability, and death to myself, my family members or others and accept sole responsibility for any injury or illness that may occur. Further, you understand and agree that this release includes any claims based on the actions, omissions, or negligence of Jazz Attack / the Philadelphia Swing and Jazz Dance Association, any third-party organization that hosts an activity at Jazz Attack / the Philadelphia Swing and Jazz Dance Association, their members, employees, volunteers, officers, and directors, whether a COVID-19 infection occurs before, during, or after participation in any activity on the premises of a JA/PSAJDA event.

8. You recognize that you are responding solely for yourself or a child under 18 years of age in your care, and not filling out this form / responding on behalf of any other parties.
