I acknowledge that participation in Beyond The Net's Summer Volleyball Clinic on July 25th, 2026 involves physical activity and carries a risk of injury. I hereby certify that I am physically fit and have no medical condition that would prevent my safe participation.

I voluntarily assume all risks associated with my participation, including but not limited to injury, illness, or property damage. I release and hold harmless the clinic organizers, coaches, sponsors, and facility owners from any and all liability, claims, demands, or causes of action arising out of or related to my participation.

I give permission for emergency medical treatment to be administered in the event of injury or illness during the clinic.

By registering for this volleyball clinic, I grant Beyond The Net Foundation, its representatives, employees, and authorized media personnel the permission to take, use, and publish photographs and/or video recordings of my child during participation in the clinic for promotional, educational, or informational purposes, including but not limited to use in brochures, flyers, social media, websites, and press releases.

I understand that:

These images/videos may be used without further notifying me.

No personal information (such as full name, address, etc.) will be used unless I provide separate written consent.

I will not receive any compensation for the use of these images or recordings.
