The Consolidated Form is essentially three permission/agreement forms in one. The form requires three electronic signatures: 1 student, and 2 parent signatures. Please read each statement carefully, and let me know if you have any questions prior to completing the form.

Photo and Video Release: I do hereby grant to Columbia County Schools the unlimited right to use and/or reproduce photographs, likenesses or the voice of my child in a legal manner and for the internal and external promotional and informational activities of Columbia County Schools. I also agree to allow my child’s work and/or photograph to be published on the Columbia County School internet web site, and CCBOE publications. I further understand that by signing this release, I waive any and all present and future compensation rights to the use of the above stated materials.* *
Uniform Contract: I agree to be responsible for all uniform parts and the care of this uniform throughout the school year. I agree that I am responsible for being dressed in complete uniform (band shirt, black crew socks, black shorts, and band/flag shoes) for all required performances. Bibbers and uniform sleeves must be hemmed to the required lengths prior to the first performance. Do NOT cut uniforms. When in uniform, you may wear light makeup. No jewelry or fingernail polish is permitted. No food or beverages are allowed in the band hallway when the uniforms are out. *
Parent Permission Form: I give permission for my child to attend all marching band-associated events in which the EHS Band participates, including football games, marching festivals/competitions, and any special performances. I understand that transportation to and from EHS is my responsibility and that transportation to and from the above mentioned events would be by CCBOE bus or charter bus and I hereby grant my permission. I understand that my student must abide by the rules and regulations set by CCBOE, EHS and the EHS Band. Any infraction will result in disciplinary actions and notification of the parent. I understand that attendance is expected at all scheduled events *
