Illinois Water Polo's Annual All-State and Hall of Fame Awards Banquet RSVP Form
Please fill this form out to reserve a spot at the Illinois Water Polo Annual All-State and Hall of Fame Awards Banquet that will take place on Wednesday, June 3rd, at Elm West Banquets (681 North Ave, Elmhurst, IL 60126). Please complete this form by 12PM on Saturday, May 30th to make an advanced reservation. See the information below about how to submit payment.

Advanced reservations are required to pay $50 per attendee. Walk-ins will be accepted on a first come, first serve basis for $60 per attendee. To RSVP in advance, please fill out the form below. With a confirmed advanced reservation, you may pay by cash (preferred), check made out to Illinois Water Polo (preferred) or credit card at the door on the day of the event. Please note that all credit card payments will be subject to a 3% service fee.
The cutoff date for advance reservations is Saturday, May 30, 2026, by 12:00PM
