Cards Given Form

Form to allow referees to submit any cards given in a game.

Game Number. *

Age Group *

Team Age Group, not the age of the player or coach

Gender *

Team Gender, not the player or coaches gender

Division *

For Grade 3/4 select the Gr3-4 option

Town *

Select the town of the player or coach that received the card or ejection

Player *

First and last name of the player AS IT APPEARS ON THE ROSTER

Number *

The uniform number of the player, enter "Coach" if it is a coach and not a player

Date *

Date the game was played

YYYY

Card Type *

Reason *

Be sure to select the appropriate code, see below for list and descriptions

Referee *

Provide the name of the referee issuing the card

Comment *

Ejections will still require a full USSF Game Report. This is all that is required for a caution. Describe the foul or misconduct that caused the player/coach to receive the card.
