2017-18 University YMCA Awards Nomination Form
All nominations must be submitted by 5 PM on Tuesday, November 15, 2017. All awards will be presented at the YMCA Annual Dinner & Auction on Saturday, April 21, 2018. Once all nominations have been received, you may be contacted by a member of the selection committee for additional information about the nominee. Please be prepared to discuss how the individual's service and leadership has resulted in positive change.
