Please complete this form if you are requesting to cancel your membership, pause your membership, or change your swimmer’s registration group to/from our Bronze Level (4 practices per month at a lower cost). This form ensures we have the correct information, required notices, and documentation so we can process your request accurately and in accordance with our team policies.

The Boerne Elite Aquatics Withdrawal Form is the ONLY method by which a swimmer can withdraw, pause, or change a registration group. Emailing or telling a coach or any other BEA employee DOES NOT meet inactivation or membership change requirements. There are NO EXCEPTIONS.
