Please complete all fields of this form- Incomplete applications will be denied. The total cost of this year’s trip is $1,800, due upon application approval. If requesting a private room please add $500. Your contact info, shirt size, dietary selections, etc. are used for our planning purposes only. 

Please note: airfare to and from Guatemala City is not included, nor are the costs of available optional excursions (typically $200-300)
