*We cannot provide scholarship assistance to cover fines for missing items or parking tickets.
*If your family qualifies for free or reduced meals, there are certain items that can be automatically waived. By answering the question below on this subject, you are agreeing to share this status with fee administrators who will apply those waivers on your behalf, automatically.
*Fee administrators are limited to the BHS principal, BHS accounting administrator and BHS counselor in charge of scholarships.

The following question is for families who have applied for and received notice that they qualify for the free or reduced meal program. If your family does not qualify for the free or reduced program, please move on to the next question.
I confirm that my family has qualified for the free or reduced meal program and consent to share this information to BHS fee administrators as defined above in order to have certain fees waived. At BHS these fees include testing fees, sports participation fees, class fees and club dues. If my family would like financial assistance for items not listed here, I understand that I must use this form to make those requests separately, as they come up.
