This form collects basic information for each LLVM contributor (as identified by the account(s) used when contributing patches and email addresses used when contributing to mailing lists, bugzilla, phabricator, or other infrastructure). This information is needed in order to proceed with relicensing.

The form also contains a link at the end to a DocuSign contract you can sign online.

We ask for your primary email address in the first field, and this is how we will contact you if we need to follow up on anything. However, we will also ask for other email addresses associated with your contributions.

While we need to ask for your name and the names of companies you may have worked for, we respect your privacy and want to be clear that this information will not be published or shared outside of the LLVM Foundation board members, administrators, lawyers, and LLVM community members directly working with the Foundation on the relicensing effort.
