Oakbrook Shadow Day Health and Information Form
Thank you for applying to Oakbrook Preparatory School. We are very excited to have your child shadow with us. Shadow Days are for students in grades 2nd through 12th. Please fill out this form prior to your child's shadow day at OPS. Please fill out one form per child.

Here's some information for your shadow day:

Dress Code
Shadowers may but are not required to follow uniform guidelines for their visit.

Monday, Tuesday, Thursday:
• Lower School uniform is a red or white collared shirt and navy pants, skirt, or shorts.
• Middle School uniform is any color collared shirt and khaki pants or skirt.
• Upper School uniform is a dress or blouse with pants or skirt for young ladies and collared shirt for young men. Ladies may wear any color pants or skirt EXCEPT denim. Boys should wear khakis.
• Tennis shoes are recommended for lower and middle school students in case the student has PE during their visit with us.

For Chapel Days (Wednesdays):
• Lower School uniform is a red or white collared shirt and navy pants, skirt, or shorts.
• Middle School uniform is a blue collared shirt and khaki pants or skirt.
• Upper School uniform is a white dress shirt, Oakbrook tie, and navy blazer or sweater vest (or cardigan for girl)
• Students ARE NOT required to wear a uniform for their shadow day but may if they choose. Tennis shoes are recommended for lower school students in case the student has PE during their visit with us.

Fridays:
• Lower school students may wear jeans and a red or white shirt.
• Middle School students may wear a collared shirts and denim jeans or skirt.
• Upper School students may wear a blouse or collared shirt and denim. Young ladies may wear dresses as long they are fingertip length and not sleeveless.

Snack/Lunch
We will treat your student to lunch during their shadow day. We will take his or her order when they arrive on campus. Please send a small snack and a water bottle.

Materials
We recommend bringing a book to read if there's quiet time as well as a notebook with paper and pen/pencil. All other materials will be provided.

Allergies/Medical Information – Please fill out the Shadower Health Form below prior to your child’s shadow day.

Assessment
• During your student's day he or she will meet with their grade level director or the Admissions Director for a preparedness assessment or placement test. This is not a PASS/FAIL test. We use it to assist us with placement.
• Please bring any previous report cards, transcripts, evaluations, or testing with you for the shadow day if you have not provided them.

Drop off/Pick Up
• Drop off and pick up will be in the Main Office. Please drop off your child at 8 a.m. on the shadow day. We will meet you and will let you know what time to return for pick up.

Please Note: OPS does NOT administer over-the-counter medications such as Tylenol, Motrin, or Benadryl. If your child needs a prescribed medication during their shadow day, please bring a doctor’s prescription and the medication in its original container. These medicines may include: INHALERS, EPI-PENS, and BEHAVIOR MEDS. *
