Dexter Mosely Act - FCSS Request to Participate in Competitive Extracurricular or Interscholastic Activities (Grades 6-12)
As a result of legislation (SB 42-Dexter Mosely Act, O.C.G.A. § 20-2-319.6) effective July 1, 2021, home study students in grades 6-12 have the option to participate in competitive extracurricular or interscholastic activities in Fannin County Schools.  Parents/guardians who desire for their home study student to participate in a competitive extracurricular or interscholastic activity with FCSS must complete this request form at least 30 days before the first day of the semester when the child will enroll and participate in the extracurricular or interscholastic activity. I understand my student must enroll in and attempt to complete at least one class offered by FCMS or FCHS in order to participate each semester in an extracurricular or interscholastic activity.  I will schedule an appointment to enroll my child in Fannin County Schools by contacting the Centralized Registrar at (706) 946-2004.  I understand my request for participation cannot move forward until my child is enrolled in the Fannin County School System. *
To enroll in school, I understand that I must present the following information/documents to the centralized registrar:  1.  A copy of my child's birth certificate; 2. A copy of my child's immunization record; 3.  Form 3300 (Eye, Ear and Dental Exam record) available from the Fannin County Health Department (706.632.3023); 4. A copy of my child's social security card; 5.  Two proofs of residency; and 6. A copy of my child's most recent home school grade report from their home school instructor. *
For athletic actitivies, I understand that after the FCSS enrollment process is complete, my child will be assigned an athlete ID and that all documentation, including a current physical and ATS paperwork, must be complete prior to participation.

For extracurricular or interscholastic activities that have a try-out process, I understand that completing the enrollment process does not guarantee that my child will make the team for which he or she is trying out.

I understand that if my student unenrolls from the Fannin County School System to enroll in a homeschool program, he/she is ineligible to participate in competitive interscholastic activities for 12 months.  This timeline is part of the Dexter Mosely Act. *
