Please complete and submit this form to request use of the church facility. Requests will be reviewed for calendar availability.
1) Non-church-ministry related events REQUIRE A $25 FEE BY THE DAY OF THE EVENT.
2) Filling out this form does not guarantee facility use until receiving a phone (or email) confirmation.
3) Weddings require additional fees, paperwork, and stipulations.

We will pay the $25 fee if applicable (for non-church-ministry related events) by the day of the event.
I am responsible to return any borrowed church keys promptly after the event.
We are responsible for all clean-up and resetting of tables, chairs, etc.
The church will NOT be held responsible for any resulting injuries from this event.
We will NOT utilize facilities, rooms, or multimedia items not specififically requested.
Never submit passwords through Google Forms.
