* Use this form to request: Unofficial Transcripts, Official Transcripts for Internships, Summer Programs, Employment, Military Programs and Scholarships

If you are requesting a transcript for college do not use this form. Transcript Requests to be sent to Colleges should go though Common App (for Common App schools) and/or Naviance (for non Common App schools).

Release of Student Records
The law requires that schools receive written permission signed by the parent/guardian before transcripts, including mid year reports and other student records can be released to third parties. By completing this form, you are verifying that it has been completed with your parent/guardian(s) approval. Please note that when you complete and submit the Guilford Park HS Current Student Transcript Request form, you are giving HCPSS permission to send the transcript through email. In addition, you are giving HCPSS staff permission to complete evaluations associated with your child’s college application and/or scholarship applications.
