Please complete this form at least the day before if you would like to come to Creekside to have lunch with your child. We will call you first thing that morning if there are too many reservations for us to accommodate. Please keep your group to 2-4 adults at the most to visit with a child at lunch.

Here are the lunch times at Creekside:
K: 10:30-11:00 AM
1st: 11:00-11:30 AM
2nd: 11:30-12 noon
3rd: 12:00-12:30 PM
4th: 12:30-1:00 PM

Please arrive at your child's lunch time. You can enter our main doors (1) and let the office know that you are here for your lunch date with your child.

Please bring your drivers license or identification so we can check you into our system. A full background check is not done for a lunch date. Our system does a quick, limited check on exclusion lists and other child safety lists.

You can bring your own lunch or reserve a school lunch in advance. A school lunch for an adult is $4.25 and please bring exact change. If you bring a lunch, please keep the packaging discreet if it is from a restaurant or fast food restaurant.

We have a guest table at each lunch for our lunch guests and their Creekside Cougar.
