Complete the following form prior to any absence from orchestra rehearsals or performances. All absence requests for performances (concerts, parades, field trips, etc.) must be received no fewer than 5 days before the anticipated absence. In the case of an unexpected absence (such as illness), please submit a form within 3 days of the absence. If you are sick the day of the performance and cannot make it, you must complete this on the day at least 12 hours prior to the performance.
