This form is only for students who are currently enrolled in the 2023/2024 ISU TRIO Pre-College Programs and have completed an ISU TRIO Annual Parental/Guardian Permission & Release form.
Additional forms may be required, please review the information carefully.
Students are responsible to provide accurate and correct information.
Failure to do so can prevent the student from attending the event(s).
Students are responsible in communicating with a contact person at their high school regarding attendance.
Students completing this form are not guaranteed to attend trip for a variety of reasons (ie: school attendance policy, limited spots, etc.)
