School Hours: 7:45 AM- 2:40 PM
Half Day Hours 7:45 AM-11:10 AM
This form will replace the need for any phone calls, emails or messages to the school/teacher about an absence or lateness and must be completed by the student's legal parent/guardian. Daily contact will be made to parents or guardians to confirm unreported absences. Absences and late arrivals will be recorded in attendance records.
This form is NOT to be used for student pick-ups. Thank you for your cooperation.Please note: Keane Elementary School administration reserves the right to contact you directly to verify the legitimacy of this form. **The information you share in this form is confidential and will only be shared with your child's teacher, and other staff on a need-to-know basis. Contact the nurse directly with the information you do not want to be shared.
To return to school after any illness your child must be:
- Fever-free and not vomiting for 24 hours
- Symptoms need to be improving
- Your child needs to be feeling well enough to be in school
If any COVID-19 symptoms are present, testing is still recommended:
According to state regulations, if your child tests positive for COVID-19 but meets the above criteria, they may return to school and a mask is recommended for 5 days.
