Declaration of Understanding and Liability Waiver (MANDATORY)

I confirm that I am wholly responsible for the child named above and the child is participating in the training arranged by Inis Jeju voluntarily. I understand that participation may involve some risk of personal injury, as do all contact sports and physical training, and that I am responsible for the actions of the child. In this regard, I agree that Inis Jeju, the committee members and coaches, and other children participating in the training shall not be liable for any injuries or for any loss or damage sustained by me during the training session.

I hereby declare that the child is in good health and fit to participate in a vigorous contact sport or physical training, and that the child is covered adequately by medical insurance.
