Artist's Laboratory Theatre Parental Consent Form
Thank you for registering your child(ren) for an Artist's Laboratory Theatre youth program! We have many options available and you will be able to select which programs you'd like to register for below. All of our programs are PAY-WHAT-YOU-CAN with suggested values. Descriptions are as follows:

SUMMER CAMP REGISTRATION IS NOW OPEN!

Artist's Lab's Summer Youth Devising Camp for ages 8 - 18 will be from July 23 - 27, 2018, Monday - Friday, 9 AM - 3 PM. The performance sharing for family and friends will be Friday, July 27 at 5:30 PM. Using ALT’s method of devised theatre, young artists will improve listening skills, develop confidence, increase cooperation, resolve conflict and encourage citizen engagement. ALT Founder and Artistic Director Erika Wilhite will co-create with young artists a short piece of theatre around a selected theme. The final piece will be shared with family and friends at the end of camp! Registration deadline is July 16.

It is highly encouraged that youth attend the majority of their selected camps. These PAY WHAT YOU CAN camps are valued at $200/week and space is LIMITED to 12 participants per camp. Once you have registered, Director of Outreach Simone Cottrell will contact you via email or phone to inquire how you would like to pay as well as sending you further information regarding the camp.  

If you have already completed a parental consent form with The Artist's Lab this year, please contact simone@artlabtheatre.com to be registered for camp.

ALT has neither the staff, facilities nor insurance to care for children when they are not in a camp program and can be responsible for students only during the hours for which they are registered. *
POLICY WAIVERS. Please mark "yes" if you agree. : 2. Students should be brought to class/camp no earlier than 10 minutes prior to the start time and picked up no later than 10 minutes following its completion time unless prior arrangements have been made for extended care. *

At the end of each camp, class,or workshop, we require a parent/guardian or a designated adult to sign-out and pick up their child. *

CHILDREN WITH SPECIAL NEEDS, INCLUDING BEHAVIORAL DISCIPLINARY ACTIONS. Please mark "yes" once you have read and understood.  : The ALT staff and teaching artists are not trained in working with children who have special needs, though some of us may have experience in working with different and mixed communities. We strive to be a safe, brave, and inclusive environment, considering the needs of everyone, including our teaching staff. This does take open and frank conversations with parents/guardians regarding the needs of the child.Please know that we will do the best we can to include your child and meet them on their level.The parents/guardians of children who are disruptive or is discovered to be high-attention needs without preparing and meeting with the Director of Outreach at least a week before the program begins, regardless of whether they have a condition or not, may be asked to leave the program as Artist's Laboratory Theatre may not be the best environment for them at this time. *

COPYRIGHT AGREEMENT: As a condition of participation, the parent grants to the Artist's Laboratory Theatre the right to photograph him/her for publication (this includes, but is not limited to, ALT brochures, postcards,website, blog, social media, and newsletters). Names may be published. By signing consent below, photographs, names,and a student’s age may be published in newspapers or by ALT's partner organizations (in print or online sources). *
