You must be a member of St. Anne's to use the following form, and you must have permission from and a direct relationship to the person for whom you're requesting prayers.

Names will prayed for privately by St. Anne's clergy and Pastoral Care Team. If requested and appropriate, they will also be read aloud in the public Prayers of the People for three weeks.
