ABOUT THE APPLICATION:
The majority of this application asks for basic identifying information or information which will help us select the volunteer opportunity which best fits your schedule and preferences. While accessible from a mobile device, this form is optimized for desktop.

After the form is submitted, STARBASE Louisiana will provided a printed copy of the completed form. Please verify the information is correct and sign in the designated spaces on the front and back of the printed permission form.

***OPTIONAL: You may choose to print the permission form and turn the signed form in to the school sponsor.
