This form must be submitted at least a week before your projected pick-up date. Your request will not be considered if the form is submitted within a week of the pick up date.

This is a REQUEST form only. Filling out this form does not guarantee the rental of any gear. Equipment reservations are not confirmed until you have received a confirmation email from the NBS Equipment Managers.

You should receive an email within two days of submitting this form regarding your rental
