Diploma Mailing Request Form
This form is required of all students completing a degree at PLNU.

Diplomas can be expected approximately 8-10 weeks AFTER the posting of a degree.

A submission of this form is only good for SIX (6) months. Once a response is older than six months, students MUST submit a new version to confirm their information.
