    In addition to this application, you must also send the following materials, either via email to mcorbin@accesscommunity.org, or through mail at the following address: ACCESS c/o Mike Corbin, NNAAC 2651 Saulino Court Dearborn, MI 48120 The network can not consider your membership if any documents are missing. – Letter from Executive Director requesting membership – Board of Directors’ letter or resolution requesting membership – Organization’s mission statement – Current list of Board of Directors – Copy of organization’s Articles of Incorporation and By-Laws – Written description of organization's history, programs, and services (1-2 pages) – If you do have a newsletter, please include a copy – If you issue an annual report, please include a copy – Copy of IRS letter designating tax exempt status – 501(c)(3); if not yet exempt, – Copy of petition for 501(c)(3) status – Copy of most recent audited financial statement; if none, copy of IRS 990 form – Two letters of recommendation from sources independent of organization (ie. funder, coalition partner, etc) – Any publications, articles, etc. that you would like to submit would be welcome -- Copy of current strategic plan, if any
