We are excited that you are interested in creating a new club here at DCHS!

Please complete this New Club Application to begin the approval process.

In addition to this form, you will also need to submit a copy of your proposed Club Constitution and budget.

Once you have completed this application, you and your Club Advisor will receive a confirmation email with a PDF attachment. Please print the PDF, sign it, have your Club Advisor sign as well, and submit it to Ms. Dawn along with a copy of your meeting minutes.

Your application will be reviewed at the next ASB Leadership Meeting (held on Mondays). If approved by ASB, it will then be sent to Mr. Hillstead for final approval.

You and your Club Advisor will be notified via email once your club has been officially approved.
