Even if you have engaged with Give-A-Kid Projects recently, it's important that you fill out this information in full so that we can provide the best support for the students on your list heading into this 2023-24 school year. All children and teens who will be attending school in Holt or Dimondale or students that live in a home with a sibling that attends a school in Holt or Dimondale (grades Pre-K thru 12th grade) are eligible for a backpack if they meet the income requirements for this program.
In addition to a backpack filled with this upcoming school year's predicted staple school supplies customized per grade, our goal is to provide every student with one new outfit to head back into the school year. This typically includes a shirt, pair of pants, and pair of shoes. When filling out your students clothing sizes, please consider what size they will be next school year, vs what sizes they wear currently.
Distribution of backpacks will take place on Thursday, August 3rd between 3 p.m. and 7 p.m. at Holt United Methodist Church on the Corner of Cedar & Aurelius in Holt, MI. Give-A-Kid Projects will reach out to you if they need to confirm any of the details you provide in this form. It will be your responsibility to provide the required proofs and pick-up your backpacks and clothing items on August 3rd. (See list of required proofs for pickup at

Note, this form accommodates up to 8 students per family. If you have more than 8 students, you'll need to fill out more than one form. Please also note that forms must be submitted by each student's custodial parent/legal guardian.
