TPHS Music Dept. Contact Form 2025-2026

1. Please fill out the Contact Form at the beginning of each school year even if you filled it out the year before.

2. Please obtain correct parent email address and cell phone number before filling out the form.

3. STUDENTS:  ENTER YOUR PERSONAL EMAIL NOT YOUR SCHOOL EMAIL. 

4. Please enter email addresses correctly.

5. Your schedule may have a different name for your music class than the one on the form.  Please select "Orchestra" if your schedule says Orchestra, Chamber Orchestra, Intermediate Orchestra, or Advanced Orchestra. Please select "Band" if your schedule says Band, Symphonic Band, or Wind Ensemble.

6. If a required field does not apply to you please enter N/A.  

7. DO NOT FILL OUT THIS FORM MORE THAN ONCE. If you need to change your info, email the changes to
music@tphsfoundation.org and we will make the changes for you.
