Letter of Information
HLTH 417 Community-Based Project: Online Questionnaire --- Letter of Information

This letter of information should give you the basic idea of what the project is about and what your participation would involve. If you would like more detail about something mentioned here, or information not included here, please ask. Please take the time to read this carefully and to understand any accompanying information. You will receive a copy of this form.

BACKGROUND

You are invited to participate in an online questionnaire being administered by students in the course HLTH 417 – Community-Based Programming and Evaluation - at Queen’s University. We are working with the KGA (Kingston Gets Active) to find out more about the assets and needs of the Kingston community. The goal of our community-based project is to use the information that we learn from you to help develop programs that may address some of the problems that you experience in seeking to maintain a healthier way of life.  We would like to ask you a series of questions about what you see as the barriers or facilitators to your participation in KGA Month activities.

Your participation is voluntary, and you are not required to participate. There are no risks or consequences to you if you do not participate. Your entitlement to services is not conditional on your participation. You will not experience any discomforts or inconveniences due to your participation. You will not incur any financial costs as a result of your participation.  

WHAT IS THE PURPOSE OF THE STUDY?

The purpose of this study is for students to learn more about community-based programming and for us to better understand the barriers and challenges that you face in leading a healthy life.  

The main goal is to use the information provided by you and others to develop a program plan that may be used by members of Kingston Gets Active (KGA) Month Group to develop grant applications or help improve programs that may work to address the issues that you raise.

WHAT WOULD I HAVE TO DO?

You are being asked to answer in an individual setting a series of questions about your feedback about the KGA Month Challenge. There will be approximately 10 questions should take 10-20 minutes to complete.

ARE THERE ANY RISKS?
Participation in the project will not place you at any risk for physical harm. If you feel uncomfortable about any questions that we may ask, feel free not to answer those questions.  

HOW WILL I BENEFIT IF I TAKE PART?

There is no direct benefit to you apart from the satisfaction you may receive from helping us better understand the barriers and challenges that you face in being physically active during COVID-19 in Kingston. However, you can enter your email address at the end of the survey for a chance to win a prize.
DO I HAVE TO PARTICIPATE?
Your participation is completely voluntary. You do not have to be involved in the questionnaire. You do not have to answer every or any of the questions included in the questionnaire. In addition, you may also choose to leave the questionnaire incomplete at any time. If any new information becomes available that may affect your participation, you will be informed as soon as possible. You may choose to withdraw from the study at any time without penalty/loss of benefits. If you do wish to withdraw, you may contact Lucie Lévesque (levesqul@queensu.ca) If you withdraw, you may request removal of all of your data from the study.

WHAT ELSE DOES MY PARTICIPATION INVOLVE?

Nothing else. Your agreement to participate in the questionnaire does not imply your agreement to participate in any other projects associated with the course HLTH 417 or Queen’s University more generally.  

WILL I BE PAID FOR PARTICIPATING?

No, unfortunately we cannot pay you for your time.

WILL MY ANSWERS BE KEPT CONFIDENTIAL?

Your comments and answers to questions will remain confidential. Only the students, teaching assistant, and instructor for the course will have access to your comments and answers. Your responses during the questionnaire will not be linked to your name. The course instructor is responsible for securely retaining any documentation and/or forms related to this course-based project for a minimum of one year.
