Rapid Damage Response (Incident) Form for Cultural Heritage Institutions

The purpose of this form is to help Texas cultural heritage institutions to protect and mitigate the loss (or further loss) of collections from the damaging effects of natural disasters and other emergencies. This form collects information similarly requested by the Heritage Emergency National Task Force (HENTF) and the National Coalition for Arts Preparedness and Emergency Response (NCAPER). The information gathered will help TX-CERA identify where assistance is sought, direct assistance where needed, and understand critical needs. This survey should take approximately 10–15 minutes to complete. This information will be shared with the Foundation for Advancement in Conservation (FAIC) National Heritage Responders (NHR), HENFT, and/or NCAPER if national assistance is necessary.

If you would like to speak with someone directly, please contact one of the following organizations:

  • Texas Collections Emergency Resource Alliance (TX-CERA) Hotline: (669)237-2243
  • Foundation for Advancement in Conservation (FAIC) National Heritage Responders (NHR) Hotline: (202)661-8068
