A 76-year-old man with ulcerative colitis on vedolizumab presents with worsening diarrhea. He was diagnosed with left-sided UC one year ago and was started mesalamine. However, shortly after, he developed a gout flare that was attributed to mesalamine, and he was switched to vedolizumab. He did well for the first 6 months, but then developed a flare of diarrhea 3 months ago requiring treatment with prednisone, with improvement. A few weeks after completing a prednisone taper, however, he again develops worsening diarrhea. Stool studies are negative for infectious, and his fecal calprotectin was elevated at 2080 ug/g. In addition to prednisone, which of the following therapies should be considered at this time? (more than one may be correct)
