2024-2025 STEM Based Preschool Registration Form

There is a $100 registration fee for EVERY STUDENT which can be paid in person, on our website mathmakessense.org (click "Pay Online") OR via Paypal: mathmakessense.pw@gmail.com

Start Date: August 12, 2024

FULL TIME tuition (Monday-Friday 7:30am-3:30pm) for our STEM based preschool is $575 per month. 
Early drop off (as early as 6:00) and/or late pickup (as late as 5:30) is available for an additional $100 per month for a total of $675 per month. 

PART TIME tuition:
2 days a week: $250/month
3 days a week: $350/month
4 days a week: $400/month

Part-time Preschool classes are Monday-Thursday from 8:45-11:15 AM .

Students will be able to participate solely in our educational rotations, perform experiments, have an outdoor break, and attend field trips. (Cost of field trips are not included in tuition)

Tuition can be paid all at once or split into two payments. There is an option to make one payment which is due on the 1st of each month OR make two payments, one on the 15th of the PREVIOUS month AND one on the 1st  of each month.  If three or four payments are needed please contact our office to set up that specific payment plan option.  

***We do no accept child care assistance at this time.***

Breakfast, Lunch, and snacks are provided. However, students may pack their own lunch/snack. We also have canteen for purchase daily during snack time. Items range from $0.50 to $1.00.  
 
There is a $100 discount for additional siblings. 

STUDENT MUST BE POTTY TRAINED OR IN THE PROCESS OF POTTY TRAINING AT HOME. PARENTS/LEGAL GUARDIANS MUST BE CONSISTENT.

VIDEO/PHOTOGRAPH RELEASE FORM (Do we have permission to take pictures and videos of your child while attending Math Makes Sense LLC Summer Camps? These pictures and videos will possibly be displayed on the internet.) I hereby grant Math Makes Sense LLC the irrevocable right and permission to use photographs and/or video recordings of my child on business websites, social media sites, in publications, promotional flyers, educational materials, derivative works, or for any other similar purpose without compensation. I understand and agree that such photographs and/or video recordings of my child may be placed on the Internet. I hereby release, acquit and forever discharge Math Makes Sense LLC, its current and former trustees, agents, officers and employees of the above-named entities from any and all claims, demands, rights, promises, damages and liabilities arising out of or in connection with the use or distribution of said photographs and/or video recordings, including but not limited to any claims for invasion of privacy, appropriation of likeness or defamation. *
I understand I will have to pay a $100 registration fee to finalize registration. This fee is non-refundable.  I also understand that payments are ALWAYS  due in advance. There is an option to make one payment which is due on the 1st of each month  OR make two payments, one on the 15th of the previous month  AND one on the 1st  of each month. If three or four payments are needed please contact our office to set up that specific payment plan option. I  understand there is a $25 late fee for each payment. I understand there is also a 5 day grace period where my child will still be allowed to attend class. However, after the 5th day my child can not attend class until payment is made.   If my child does not start within a week of registering (starting August 12, 2024), I understand my child's spot may be taken if someone else registers and starts their child prior to my child.                                                                                                    *
