Thank you for your interest in onboarding to Imunis with Central Health.
Important points about signing up:
1. At the current time, Imunis is only available in English language.
2. At the current time, records from Central Health's Duddell Street clinic will automatically be transferred to enrolled patients via Imunis. Records from Southside or Island Health clinics will only be transferred if patients confirm having received a vaccination at one or both clinic sites as part of the enrolment process.
3. The person managing health information and family members on the Imunis app is referred to as the 'Imunis Account Holder'. The Imunis Account Holder must be over 18 years of age. Enrolled Imunis Account Holders will be able to receive their vaccine records as held with Central Health via the Imunis app.
4. Please indicate on the form whether or not you are a registered patient. Unregistered patients completing the form will be registered with Central Health. Central Health may need to request additional information in order to register you.
5. Children under 18 years of age are managed as a 'Profile' by an Imunis Account Holder. You may add details of children in your care that you would like to add to your account in Section 3 onwards of this form. A child's records may only be connected with one Imunis Account Holder for onboarding, after which the Account Holder may use the sharing functionality to share records with other carers. In some cases, Central Health may need additional information to verify a carer/child relationship, in which case they will contact you.
6. Anyone over 18 years of age must have their own Imunis Account.
7. Imunis may contact you by email to request feedback about your user experience, which will be used to help improve the service.

The information collected via this form is used for patient identification and records release purposes, and to enable Imunis to provide appropriate vaccine schedule information to you. It will be stored securely and deleted once onboarding is complete.
