St. Thomas More Catholic High School  Community Service Volunteer Request Form: "God's Servant First"
This form is intended for use of organizations to request volunteers for service in the community.  Please fill out this form so that we can better accommodate your request for student volunteers. We ask that you give a two weeks notice, minimum to best help with your request.  The service that is being requested must go hand in hand with the mission of our school: 

St. Thomas More Catholic High School educates students to be seekers of truth, individuals of character and “God’s servant first.”

These are examples of acceptable Community Service Activities, but are not limited to:  Clothing Drive, Clean-Up Efforts, Local Charities, Nursing Homes, Volunteering at a non-profit agency, Church, etc. 

Once all information is received, the service coordinator for St. Thomas More High School will send the information to the sponsor/coaches of our groups and then reply to the person requesting service. It will be the responsibility of the organization to contact those persons in charge and organize volunteers, times, etc. through the adult sponsors. 

NOTE:  Please fill out this form in its entirety, and within 12 to 24-hours the service coordinator will contact the supervisor to confirm the service request.
