COVID-19 Smith Hill Help
During this difficult time, we understand many of our elderly and immune compromised neighbors are stuck at home and many of our service industry and hospitality worker neighbors are out of work with no security net. This form will serve as a way to connect those who need emergency supplies and services to Councilor Kat Kerwin, the Smith Hill CDC, and the Smith Hill Partner Initiative who will work to connect neighbors to supplies.
This is a cooperative, free service organized rapidly by a multitude of Ward 12 neighborhood non-profits, elected officials, and individual volunteers.

If you are in need, what do you need?
Donated food or donated basic household supplies delivered to my home
Someone to pick up and deliver prescriptions, groceries, or supplies to your home
Transportation to or from a non-emergency medical appointment
Someone to talk to by phone or to check-in
Do you have resources you are willing to give?
Provide check in calls to those in isolation
Never submit passwords through Google Forms.
