Please fill out the following form to complete your student's withdrawal from their dance class(es). The form must be submitted before the 15th of the current month, or you are financially responsible for the full tuition fee for the following month. If they are in more than one class, please list each class you would like to withdraw from. If you would like to withdraw more than one student, please complete a separate form for each child.
