Preregister for Smith Preschool Storytime
----->Please complete a form for EACH adult who will be attending Smith Preschool Story Time.   All forms must be submitted by 3:00 PM on the Tuesday BEFORE the scheduled story time.  Anyone not on the list will need to sign in using the Hall Pass System in the Front Office.  Each name submitted using this form must have previously had their driver's license entered in to the Hall Pass System

----->Upon conclusion of the story time, please exit through the front office.  Staff members will be collecting your visitor pass and will check you out of the system.  Visitors are not allowed to follow students into PODs or classrooms.
