This is the official data request form of the Monitoring and Evaluation Division (MED). This form aims to streamline the process of data requests.
Please note that the standard processing time* of data request depends on the availability and complexity of the requested data based on R.A. 11032 (ARTA) :
3 Days - Simple Transactions (Latest Approved Cumulative Data)
7 Days - Complex Transactions (Latest Approved Disaggregated Data)
20 Days - Highly Technical Transactions (Unprocessed/Raw Data)
The following are the regular reports which the MED can provide depending on the latest approved data:
1. National ID Weekly Report - this report reflects the latest cumulative figures on major indicators of the PhilSys registration (Registration, PSN Generation, Dispatched National ID Cards, Issued Printed National ID in paper form, Downloaded National ID, and Delivered National ID Cards).
2. National ID Grand Table - this monthly report reflects the city/municipal level disaggregation of the Registration, PSN Generation, Dispatched National ID Cards, Delivered National ID Cards, and Issued Printed National ID in paper form(Total cumulative downloaded National ID is only available starting December 2022 report onwards).
If deemed necessary, the MED will clarify the request to specify the information requested.
For other reports not maintained by the MED, MED will advice the requestor to coordinate with the appropriate division/service who may provide the data request.
*The standard processing time indicated is the maximum number of days per type of transaction.
All collected information will be used ONLY for statistical purposes pursuant to Republic Act (RA) No. 10625 (Philippine Statistical Act of 2013), RA No. 11055 (Philippine Identification System Act of 2018) and RA No. 10173 (Data Privacy Act of 2012).
