The purpose of this evaluation is to provide confidential feedback to clinical teachers. The information provided will provide a basis for rewarding those faculty members who are good teachers, for assisting faculty members to become better teachers, and to help avoid situations where learners are placed in a teaching environment with an individual who does not enjoy teaching. All information is CONFIDENTIAL and will only be used collectively.  You must submit a separate form for EACH unit your supervisor (even if they supervised you for multiple units).
