FitWell 4 U Request Form
FitWell 4 You is a service that brings fitness and wellness to your department, club or building. We bring the equipment, presentation and facilitator; you bring the people! Please complete the form at least 2 weeks prior to your expected event date with as much information as possible to assist us in serving you and your group. Reservations are filled based on facilitator availability and are not guaranteed.

$25 for ASU on-campus residential halls, student organizations and off-campus residential communities
$40 for faculty/staff organizations
