Rotary Club of Sun Lakes - Nomination Form

Any faculty, staff, or parent can nominate a CUSD teacher simply by filling out and submitting a nomination form. Nominees should use the judging criteria, below, and provide a narrative, with specific examples, that supports checked criteria.
Sun Lakes Rotary Club will review all nomination submissions, score them, and select the recipient to be awarded, monthly.
