This form is sent to the Vice President of Enrollment Management and the Vice President for Finance and Administration for review of your complaint. Outlined below is the procedure for all Formal Complaints. If you have any questions please contact the Office of the Provost at 563-387-1005. Prior to submitting a formal complaint, a student should make every attempt to resolve disagreements with the person(s), department(s), or office(s) involved. Students will be notified of the institution’s response to the complaint.
