I, the undersigned, acknowledge that I am aware and understand that obtaining permanent jewelry (“Activity”) requires the procedure of welding jewelry, and such procedure involves some level of risk.

I acknowledge that I am voluntarily participating in the aforementioned activity entirely at my own risk. I confirm that I am at least 14 years of age or the age of majority to consent to the Activity.

I understand not to look directly at the "spark" during the welding process. Doing so could severely affect my eyesight. I understand that any sudden movement could cause the welder to scratch or burn my skin. I acknowledge that I must sit still and not look directly at my permanent jewelry as it is being welded.

I acknowledge that it is my responsibility for maintenance and care of the permanent jewelry after the procedure and that Lavish Everlasting LLC will not be liable for any loss of or damage to such jewelry that may occur.

I confirm that I am in good health and proper physical condition and do not have any medical or other conditions including allergies that would impair my ability to participate in the Activity. I acknowledge that Lavish Everlasting LLC is relying on these statements to allow me to participate in the Activity.

I hereby waive and release any and all claims against Lavish Everlasting LLC and its officers, directors, managers, employees, agents, affiliates, successors, and assigns (collectively, “Releasees”) arising out of or in connection with my participation in the Activity.

I further agree to indemnify, defend and hold harmless Lavish Everlasting LLC against any and all claims, suits or actions of any kind whatsoever for liability, damages, compensation or otherwise brought by me or anyone on my behalf, including attorney's fees and any related costs.

This Release constitutes the sole and entire agreement of Lavish Everlasting LLC and me with respect to the subject matter contained herein and supersedes all prior and contemporaneous understandings, agreements, representations, and warranties, both written and oral, with respect to such subject matter. If any part of this Release is declared invalid, illegal, or unenforceable, it will not affect the rest of the Release. This Release is binding on Lavish Everlasting LLC and me and our respective successors and assigns. All matters related to this Release will be governed by United States laws and any claims or legal actions related to this Release can only be brought in United States courts.

BY SIGNING, I ACKNOWLEDGE THAT I HAVE READ AND UNDERSTOOD ALL OF THE TERMS OF THIS RELEASE AND THAT I AM VOLUNTARILY GIVING UP SUBSTANTIAL LEGAL RIGHTS, INCLUDING THE RIGHT TO SUE LAVISH EVERLASTING LLC.

After Care Instructions:

Permanent Jewelry is like any other piece of jewelry, it is not indestructible so please treat it with care!

Avoid: 

Getting chains caught or pulled as this may cause them to stretch or break; direct contact with any abrasive or hard chemicals; applying perfume, lotions and sunscreen directly on the jewelry

Note: 

Thoroughly dry jewelry when wet: pools and hot tubs may discolor jewelry; sterling silver May naturally oxidize over time. 

To clean: 

Wash with a gentle soap and brush softly with a spooly or soft toothbrush. Dry thoroughly and polish with a jewelry cloth.

We ensure the integrity of our weld before our appointment is complete, however, things happen! Although our pieces are designed to be permanent, that UNFORTUNATELY does not mean they are unbreakable. if your forever item breaks at the weld in the first 14 days and you contact us within that timeframe we will weld it back on for free.

While these bracelets are designed to withstand daily wear, they do have their limits. Avoid pulling, tugging, or getting your bracelet caught on anything to avoid breaking the chain.

If for any medical reason you need to remove your bracelet, simply cut the jump ring where the chain is welded with kitchen scissors.

If you need your chain welded again for any reason, there is a $20 fee to reweld.
